We present the case of a 23-month-old male with a one-year history of intermittent abdominal pain who presented for aroutine well-child visit and was incidentally noted to have a large abdominal mass and regression of developmentalmilestones. Upon further investigation, the child was diagnosed with Wilms Tumor (WT) with metastases to the periaortic lymph nodes and bilateral lungs. Initial management included left radical nephrectomy, lymphadenectomy, andmediport placement for chemotherapy infusions. Tumor cytogenetic report was positive for loss of heterozygosity at11p15 region and pathology demonstrated blastema predominant WT. Treatment decision was based on a therapeuticclinical trial (AREN0534) outlined by the Children’s Oncology Group and the patient was placed on a 28-week chemotherapy regimen, including vincristine, dactinomycin, doxorubicin, cyclophosphamide, carboplatin, and etoposide in conjunction with radiation. MRI of the head showed no evidence of metastasis, however it demonstrated multiple areas ofinfarction in the distribution of the posterior cerebral artery (PCA). Coagulation studies were performed to evaluate possible underlying causes and the patient was found to be heterozygous for Factor V Leiden (FVL) mutation. While FVL isassociated with an increased risk of venous thromboembolic (VTE) events, arterial thromboembolic events are generallyrare. In this report, we explore potential etiologies for the unique presentation of brain infarction in a pediatric patientwith WT.

Background: Mediastinal hemangiomas are extremely rare benign vascular neoplasms, accounting for less than 0.5% of all diagnosed mediastinal masses. Primary origin from or direct involvement of the walls of the right superior vena cava (SVC) is exceptionally uncommon. We present the case of a 31-year-old man with a 6-month history of recurrent upper respiratory tract infections. Chest radiography showed displacement of the tracheal midline, and noncontrast computed tomography demonstrated a large anterior mediastinal tumor arising from the right SVC. Complete surgical excision was performed through an open transthoracic approach to ensure safe vascular control. Intraoperative findings included abundant thymic fat, multiple dense fibrous adhesions, and a 5 × 6 cm tumor involving the superior jugular vein, right pleura, and pericardium, as well as periaortic neovascularization. Definitive histopathologic examination confirmed a cavernous hemangioma with dystrophic calcification arising in residual thymic and adipose tissue and showed positive surgical margins (R1 resection) in the specimens from the mediastinal tumor, the SVC extension, and the periaortic tissue. Because of the absence of a true fibrous capsule and the lesion’s intimate fusion with the adventitia of the great vessels, achieving microscopically negative margins (R0) is highly challenging when preservation of vascular structural integrity is prioritized. Therefore, after an R1 resection, although the benign nature of the lesion precludes metastatic potential, strict long-term clinical and imaging surveillance with serial CT scans is required to monitor for possible local recurrence.

We establish a universal mathematical theorem and verified computational suite for aligning arbitrary physical noise to a common logical action across general stabilizer architectures. For any [[n,k,d]] stabilizer code encoding a K=2^k dimensional logical system via isometry W: \mathcal{H}_L \rightarrow \mathcal{H}_P, and defining the correction radius t = \lfloor(d-1)/2\rfloor, every completely positive trace-preserving (CPTP) Stinespring noise process supported on any selected set of at most t physical qubits is proved to be exactly correctable.
Rather than presupposing pre-aligned Kraus branches, the framework expands local physical operators in the complete Pauli basis, derives the Knill-Laflamme scalar matrix from code distance, and Gram-diagonalizes the resulting error-image matrix.
